Misfires are the only form of malfunction currently in the game, and they are caused by the AMMO. Should be pretty obvious considering the bolt cycled and the round chambered that it wasn't any kind of jam. It even says MISFIRE in for you in the bottom right. "Different ammo now has increased or decreased chances of misfires"

If it is a double feed the extractor isn't going to grab 2 separate rounds and try ejecting them both individually. I didn't see the round sticking out on the first viewing. Do NOT screw the head up tight to the screw hole. Once the epoxy has set, apply a vice grip to the screw head, and rotate it out. That should give you a new set of threads that will last a while.
